Decision pathways

What decision is your leadership team facing?

Each pathway outlines the decision, common leadership scenarios, and relevant work to help you explore the next step.

Examples of prior health-system work reflect previous employment roles, not CIRRANE client engagements. They describe relevant operating experience and do not imply employer endorsement or promise comparable results.

01 · Essential services

Essential service continuity or redesign

Decide what should remain local, change, or move to a different care model. Compare community need, clinical coverage, workforce, financial contribution, and partnership options together.

Leaders may be weighing

  • Physician coverage is changing and leaders must protect continuity.
  • A service line is under strain from volume, staffing, or margin pressure.
  • A partner or professional services agreement (PSA) is on the table and responsibilities need definition.

Relevant work and CIRRANE’s approach

Relevant work

Following an orthopedic-practice divestiture, the service-line director aligned physicians and hospital operations, preserving more than 1,200 surgical cases and $11 million in revenue while maintaining year-round emergency department call coverage.

CIRRANE’s approach

Compare feasible options against community need, clinical requirements, physician coverage, workforce, capacity, capital, and operating risk; prepare a recommendation for senior leadership consideration.

02 · Access and site of care

Local access and site-of-care strategy

Determine where care can be delivered safely and sustainably while preserving meaningful local access. Weigh patient travel, provider capacity, facilities, capital, and referral relationships.

Leaders may be weighing

  • Outpatient volumes are shifting and the current footprint may no longer fit.
  • Leaders are weighing local versus regional care for selected services.
  • An ambulatory surgery center (ASC) or partner site is proposed and its role needs testing.

Relevant work and CIRRANE’s approach

Relevant work

Previous employment work included evaluating service growth, capital investments, facilities, and physician recruitment across hospital and regional-system settings.

CIRRANE’s approach

Assess hospital, outpatient, partnership, and digitally supported options, then define the clinical, staffing, governance, and operating conditions required for the chosen model.

03 · Virtual-care capability

Virtual-care capability and operating models

Define which virtual capabilities solve a local clinical or staffing need, and how they fit into daily operations. Test the staffing, technology, governance, escalation, and funding required to sustain them.

Leaders may be weighing

  • Virtual care or specialty coverage is being considered to extend local teams.
  • A technology investment is proposed without an agreed clinical workflow.
  • Current telehealth programs vary by site and leaders need a scalable model.

Relevant work and CIRRANE’s approach

Relevant work

Previous employment work included enterprise virtual-care leadership across a six-hospital system and funding secured to support more than 100 virtual-care locations. The work included virtual care from concept through stabilization at two hospitals and integration of virtual care into inpatient, outpatient, and urgent-care services.

CIRRANE’s approach

Assess where virtual care can expand access and support revenue, then define the clinical model, reimbursement requirements, staffing, technology, and workflows needed to operate it.

04 · Execution readiness

Operational readiness and implementation planning

Before committing resources, identify whether the organization has the ownership, people, sequence, and capacity to act. Turn the chosen direction into clear decisions, accountable leaders, and near-term steps.

Leaders may be weighing

  • A strategic priority has board support but no executable path.
  • Several initiatives compete for the same people or capital and sequencing is unclear.
  • Prior change efforts stalled because owners, measures, or dependencies were missing.

Relevant work and CIRRANE’s approach

Relevant work

Previous employment work included clinical-systems readiness and business-continuity support for a new hospital opening, along with operational-readiness work supporting DNV Comprehensive Stroke Center (CSC) and Advanced Spine Surgery certification reviews.

CIRRANE’s approach

Work alongside department leaders to clarify responsibilities, coordinate decisions, identify resource needs, and prepare the organization to carry its strategic priority forward.
